MEDICAL CONDITIONS

If a student has a life-threatening allergy and/or disease, the parent/guardian is encouraged to complete and return the following WCASD forms to ensure the release of medical information to bus drivers and appropriate school personnel.

 ASSESSMENT FORMS

The WCASD has assessment forms for the physician to provide specific orders in managing a student 's diagnosis of asthma and seizure disorders.  These guidelines allow the physician, school nurse, student and parents/guardians to work cooperatively in providing optimum care for the student in the school setting.
